LM5177: Nothing Happening on power up?

I used the ti webdesigner to create the attached schematic. I designed it to allow 12v input with 57v output. When I power the board and apply 12v I see barely 1 volt output. What is wrong on the schematic to prevent start up? Additionally the total power draw is about 10 mA.

    thank you for using the E2E forum.

    As the average current limit is not used please connect IMONOUT to VCC to disable the limiter or just use an Resistor for Current Monitor configuration.

    For further debugging please check following signals:

    - VIN stable

    - SS ramping up

    - SW1 and SW2 - is the controller starting operation with switching this signals

    - COMP pin
