We have had cases where abnormal voltage has occurred when the input voltage fluctuates within the rated voltage when EN is ON or OFF.

    Would you happen to have schematics and scope shots of this happening?

    are you able to verify that the device is soldered on correctly? Are you able to verify that the flux has been cleaned off the board, minimizing the chances for a signal to couple onto EN?
