Hi JH,
1) VLOOP gain of 8 sets the voltage loop bandwidth too high for a 650kHz switching frequency. I would suggest changing to Comp Code 28 to set the ILOOP gain to 7 and VLOOP gain to 2. To do this, change MSEL pinstrap resistor Rtop (R28) to 26.1k and Rbot (R34) to 46.4k.
2) AGND and PGND are all tied together on the same net on the schematic. Make sure in the layout, AGND is connected to PGND at a single point at the thermal pad.
