What are the consequences of having VIN > Vbias? I.e. Vbias=3.3V VIN=4V?

Keeping VBIAS<VIN can result the device not to turn on since charge pump in this device is not strong.
In your example it can turn on but RDSon will be high.
you won't get the optimal RDSon as mentioned in datasheet.
You can connect VIN to VBIAS.
