I get an error in webench when I use 12V input and 5V output for 15A with this device.
Is it my understanding that this device is limited to 12A with 3~4V input and 15A with 12V input regardless of the output?

Yes, if the Vin is 3V-4V the device is limited to 12 A without external bias. If the device has 12 Vin, then it can go up to 15 A.
There might be some cases that WEBENCH doesn't have, but the excel calculator is also a good way to verify a design.
