What is battery voltage? 12V?

Adapter is not present, is that correct?

This is only happening on some of your boards?

The full battery voltage is 12.6V, and the input voltage during the test is 12.6V

Adapter not connected

It occurs in a small percentage of boards, about 1%

It doesn't look like your ACFETs are rated for a gate drive of 12.6V.

The ACDRV is an output, so I don't understand how the voltage would decrease.

I also notice in your scheamtic you have R829=0ohm, this should be 10 ohms.

Can you also remove the 100uF from the input.
