LMR51450: Input blown

Hello everyone,

We’ve been using the LMR51450FNDRRR switch-mode regulator in a series of PCBs that we manufacture. The regulator has been working perfectly in all of them, except for two boards where we encountered a serious issue: the input side of the IC failed catastrophically. Upon analysis of the burnt IC, it seems that only the input stage was damaged, while the output stage remained intact.

We are regulating 24V down to 5V. Could anyone provide insights into what might cause the input of the IC to fail in such a way?

We've thoroughly checked for short circuits, both on the 24V input and the 5V output side, but found nothing conclusive. Any advice or guidance would be appreciated.
