What is the max load current in your circuit? Kindly fill the SLVC452 Calculation tool | TI.com calculator for detailed review and share it on this thread.

Do you have pullup on PWRGD pin. The PWRGD pin is an open drain output. A pull-up resistor of 1 kΩ to a voltage source that is 5.5 V or less is recommended.
