LM70840-Q1: Overheating to over 100 degrees C

Tool/software:

Implemented a design from WEBENCH using the LM70840-Q1 with the following parameters...

VinMin = 46.0V
VinMax = 57.0V
Vout = 30.0V
Iout = 4.0A

With minimal load, IC overheats to over 100 degrees C.  Connected BIAS pin to VOUT instead of AGND and IC runs a little cooler, but still around 80 degrees C.  Also tried connecting an external 12V source to BIAS pin, IC still run at around 80 degrees C.

Why is this chip getting so hot?!!
