TPS61022: Device damaged with SW pin shorted with GND

Tool/software:

Product: USB power source.

Power supply: 4V(cell)

Problem: the device TPS61002 was always damaged during testing, especially, it's easier to damage in a case that to power on/off with(recycle to power on and off, test with power supply) loading 3A.

               symptom is the SW pin short with GND.

SCH: 

ever tried to adjust the inductor to 1uH, the problem was still going with.

Question:

1. any problem on the layout?

2. if the spike voltage on SW exceeds 7V, could it be a risk to the device? any idea can eliminate or lower the spike voltage? actually, we tried to adjust the parameter of the inductor, RC circuit and add more caps here, it seems it's unhelpful for it.
