The top marking should be 41A for the 3.3Vout.

The mark may be on the bottom.

In any case, it sounds like the device is the 5V version rather than the 3.3V version.

You may need to order the 3.3V version of the regulator and swap that into the EVM.

There is no mark on the bottom either (I have spare parts from cut-tape that I can view).  No markings at all.  The DigiKey label has the correct 3V3 MPN but the actual parts are 5V.  This is a shame because all of the 3V3 loads connected to this regulator were damaged because 5V exceeds the absolute-max rating of these parts (one of the parts is a $6K module !).  Not sure if TI warranties this type of failure mode or not.

Can you send us a clear picture of both the top and bottom of the device and of the label on the bag.

I ended up isolating the load, and the output is now 3V3 volts.   Something on the load is back feeding 5V, so I have further troubleshooting to do.

As of this thread, consider it resolved.
