If the FUSE pin is not used than you would connect it to VSS.

It is not recommend to not use the CHG pin but you could leave the CHG pin floating.

Yes, please into the EVM and datasheet to see the typical schematics and layout for the BQ40Z80. You should be able to take heavy Influence from them.

Hello Ryker,
Sorry for the confusion but if you are designing a external balance circuit using a TI gauge BQ40Z80 I would rely on the image below, seen located in the previously mention post. As a designer of the PCB your team would have to modify some aspect of this schematic such as connector for load/battery, select LED to be on the board, determine if the FETs are right for your application and etc. If this project is still in early development of the hardware stage I would recommend using the EVM to do a proof of concept of the system.
I believe you are asking about how to wake up from shut down mode and I would review the Datasheet, User guide, and Application note under the Technical documentation. I also attached the links below to some of them below aswell.
