BQ24640: Low charging current

I am trying to use the BQ24640RVAR to charge a super capacitor rated for 9000F at 4V and 7A.  The input is 12V.  The super capacitor is currently at 1.8V, but the output current to the capacitor is extremely low. I will attach the schematic for the circuit I am using, based on the typical application from the datasheet. 

Probing the gate for the high side transistor gives me extremely short voltage spikes with a lot of ringing afterward.

Any insights on where to begin troubleshooting?
