Background:1.total 215104pcs were used, and no similar faults were found before! 2.1pcs failed samples till now.
case study:
At room temperature, TPS54061QDRBRQ1 PIN2 VCC are powered on with a 12V power supply. Point B outputs a 12V pulse waveform, as shown in the waveform of channel 1 in the above figure, and point A outputs 5V, it means it work well;
At a temperature of 80-120 ℃, when TPS54061QDRBRQ1PIN2 VCC is powered on again, the voltage at point B is 0V and point A is also 0V. After the temperature returns to room temperature, the waveform returns to normal.

So, It seem that the environment temperature impact on the chip function& performance.
