TLV61048: TLV61048 USE?

Tool/software:

       Using TLV61048, the input voltage is 3.3V, the output is 5V, 9V, 12V, according to the datasheet R1 select 698K, and then the different circuit output three different voltages, after testing found that the board has just been powered on and the output is normal, and then the voltage will gradually decrease, 12V will be reduced to about 11.5V. 9V will drop to about 8.65V, 5V will drop to about 4.9V; After adjusting L4 inductance, or changing the resistance, there will be a normal working voltage at the moment of just powering on, and then the voltage decreases. What may be the cause of this problem? Could you please help to see if the schematic design is reasonable and there is something that needs to be optimized?
