TPS54336A: Power supply chip's lower MOSFET burned out

We are using the TPS54336ADDA component, and we have encountered 3 failures. The failure symptom is a short circuit between pins 3, 4, and 9, with the failure analysis pointing to overcurrent damage of the lower MOSFET. The input side of the chip has a DC-DC isolated power supply, providing 15V to the TPS54336. The output load is relatively small, and we are using ceramic capacitors instead of electrolytic capacitors on the output side. The peripheral circuit of the power supply is as follows

the waveform measured at pin 3 on a normal board is shown below. Could you please help analyze whether the SW waveform is normal?
