TPS1211-Q1: Abnormal output

Hi expert,

My customer found the output voltage can't stable and dropping slowly with OCP fault, but the current is lower than 10A. Below is the schematic:

The voltage of pins:

Pin1: 2V

Pin2: 10V

CS+, CS-: 8V, which is dropped slowly when MOS opened 

FLT_I: low level

FLT_T: High level

Below is the waveform of pins: Yellow-- PU, Blue -- BST, Red-- PD, Green-- Input

In below waveform, Vin=12V, Vout= 3.12V, and then drop. The max Iout= 4.26A, FLT_I is low level.

Now I want to confirm 2 questions:

1. Are there errors in the schematic?

2. It seems OCP always enabled. I don't know why because the current is lower than 20A. Do you have any insight to solve the problem?

    1. Can you clarify what you are using for your load condition? Is this an eload in constant resistance mode? 
    2. Do you have the FLT pins pulled up correctly? 
    3. Is your input supply stable during this time? In the first diagram you say CS+/- is 8V. Is your Vin 12V at this time?
