LM5176: Stability issue

Dear Team,

i have designed a DC-DC converter using LM5176 based on Webench - though some parts are altered. My input voltage is ranging from 9 to 13 V and the output is fixed at 12 V. 

The design is built, however the system is unable to produce 12 V output despite stable 10 V input. It outputs around 4 V with no load. The VCC is stable at 6.7 V. Other signals are measured using oscilloscope.

As for now, I haven't been able to test the design on another board. Could you please review my schematic and signal captures, whether it's a design problem, or damaged chip?
