TPS564242: Buck Converter Vout is equal to Vin

First time posting here, and electrical power design is not my specialty (as will be very obvious!). I'm a software and systems expert, usually... So this may be a very dumb question.

I'm trying to implement a multi-purpose board that takes in 12v automotive battery power, and then steps it down to 5v. I've also added in some other components (such as a current monitor and temperature sensor). I used web bench for the design, and it had some errors that i submitted a ticket for. Based on that response, i thought had caught all the issues. However, when i received the board back from fab and assembly, Vout is always equal to Vin.  I also read through converter documentation, and i thought i had followed it pretty well. The only thing i can think of is that the Rfbt and Rfbb are close to SW.

Can someone help me troubleshoot? Schematics are attached.
