TPS6593-Q1: PMIC Issues with AM62A7 Custom Board

We have built a custom board using AM62A and TPS65931211RWERQ1 PMIC IC.
However, we're currently facing some reliability issues, which is why I'm reaching out to you.
In a lot of 10 proto boards, 4 boards are working fine with PMIC and the rest 4 have PMIC issues. 
On those four boards, GPIO4 does not stay high — it gives a pulse for a few milliseconds and then remains low. This GPIO is responsible for enabling switches and other voltages. As a result, LDO3 and LDO4 are receiving 1.8V, but the other core voltages are not being released by the PMIC.
We have attached the PMIC and Power section of the board for your reference. Please let us know the solution.

    Since these are proto boards and some work fine and other don't, let's try to take a closer look at a single board.

    Now with that in mind, from looking over at the section relating to the PMIC I don't see any glaring errors.

    Do you have any scope shots in the order of ms or maybe a 30ms capture at most, I'll leave that at your discretion for now.

    Triggering on VCC_3V3_MAIN or PMIC_nPWRON signal, if the issue is further down the power sequence please still trigger off of Ch1. and change Ch3 & Ch4 to work your way down the sequence.

    Ch1. VCC_3V3_MAIN (at 2V7 rise) or PMIC_nPWRON (at 1V26 rise)

    Ch2. VCC_3V3_SYS_EN

    Ch3. VCC_3V3_SYS

    Ch4. VCC1V8_SYS

    If possible could you also provide a register dump after the error in power has occur through an I2C device.

    The registers I'm interested on device 0x48 are:

    0x5A to 0x6C, these are interrupt registers which would reveal more about the cause of failure this would be the quickest way to rule many things out.
