TPS2663: dVdT Capacitor Remains Low and then randomly switches on

Tool/software:

We have a TPS2663 as a switch on a 12V channel limiting the channel to 3A. We have a 100 nF dVdT capacitor.

We checked all the following reasons why the dVdT remains low:

  • TSD - Temperature - Board temperature is at 40 °C
  • UVLOb - 2.88V which is above the 1.2 V limit
  • PORb - P_IN Voltage is 12V which is above the 4.3 V limit
  • SHDNb - 2.9 V which is above the 2V limit

but they all are in their correct ranges. 

Could the charge pump be damaged? 

Could the 14 ohm pull down MOSFET be damaged?

What could cause such damage?
