The schematic looks referring to PMP22477 with op-amps based power limiting function removed.
It seems you do not have 2nd side 12V adapter oring and PoE flyback voltage output low-down. If so you can remove the below circuit and make PPR58 4.99k --> ~4.37k. Other PoE circuit looks good to me.
