The 20 kOhm resistor is needed for the device to be able to detect an open load event by measuring (Vs - Vout) < V(ol,off)
if that resistor is not their it will not be able to determine if there is an open load
2)
that load seems fine

We don't have a recommended D and TVS as it can depend based on the inductance of your load however we do have the calculations you can do to determine the correct diode and TVS to pick in section 8.3.4 of the data sheet.
