I we were quite focused on what could be wrong with our design, trying a lot of tests, and I can say we are not violating any MAX-ABS ratings, we have ~3% random failure in manufacturing and I would appreciate bit of private conversation as I won't be able to share as much here publicly
We identified that this gate driver is dying and also FETs are sometimes dying.
