Damaged LMZ12001

Hi,

I use LMZ12001 for regulating system power --> Vin = 12V, Vout = 5V / 200mA. Input 12V is regulated voltage from external power supply. 

We produced about 50 pcs. of boards with this switcher and in first 2 weeks about 10 of them are damaged. In most cases was problem in LMZ12001. We found 2 odd problems (only on damaged boards):

- first problem was at pin 3 (ENABLE) which we don't use on our board, but have only 2-40 ohms to GND,

- and some of them have critical resistance (only few ohms) between input and output.

Any idea what is wrong?

    Sorry for the delay we have been discussing your problem, and we have a few questions

    1) Are the failed parts initially working and then eventually fail, or are they failing immediately after being built?

    2) Are you hot plugging the module from a wall-adapter?

    3) Are there any ceramic capacitors on the input of the LMZ12001?  We recommend some ceramic capacitance on the input to the device because the lower inductance will decrease the peak voltage seen on the internal switching node of the device.
