TPS7A3301RGW regulator reliability

I just want to know if anybody have the same TPS7A3301RGW reliability problem as me.

The circuit is very simple, same as the official datasheet.

Everything is working fine initially until I use my finger to touch the surface mount 1uf NR ceramic cap, the input cap is very near the NR cap, my finger may also touch it the same time. anyway, The circuit stop work immediately, TPS7A3301RGW die.

The second problem is reverse protection. I know the datasheet do not include it, but using a 22uf output ceramic cap and no input cap, the chip also die immediately when power off.

All the above test condition is -20V input and -12V output

The chip are sample from TI directly, is this just a individual batch problem or it's normal?
