Need reference design for 150W AC-DC adapter

Hi,

I am looking for a reference design of 150W AC-DC adapter. My requirements are as follows.

1) Vout=12V, Iout(max)=12A

2) Efficiencty larger than 80%

3) Low cost

Can anybody recommend for me? Thanks
