BQ24703 - kills DC/DCs connected to system output

I am using the BQ24703 exactly as shown in the datasheet example.

when I received my new boards, the 3.3V and 5.0V DC/DCs connected to the system output worked fine until I set up the battery charger.

As soon as the input voltage increased enough to be detected by the ACDET input, the charger switches on the FET and I get a large current load on my bench supply and my output DC/DCs burn up.  The DC/DCs are connected to the system output (12.0V  in this case).  No battery is connected, yet (I'm worried after smoking my DC/DCs) and I've tried load and no load, same thing happens.

Any suggestions on where to start looking would be great.
