TPS65070 power up

According to the State Machine if Figure 33, if I have both the AC and USB pins each pulled low with 100k resistors and set POWER_ON to '0' from '1', the PMIC should go to the "Power OFF, SYS OFF" state. Unfortunately it is going to the "POWER OFF, SYS ON" state. This would not be a big deal, but for some reason the next time i bring 'PB_IN' low the PMIC does not boot. I am using a single cell Li-ion battery that is always connected. To get the device to work I have to remove the battery, set my switch to the off position, put the battery back in and finally switch to on.
