TPS54620 Failure

I have a project that uses the TPS54620. I'm having every single one of these fail whenever break the input voltage with a switch, wire on a terminal, or push button and reapply it it dies. The inputs have been from 10v-15v, from lead acid batteries, bench top supplies, switchmode powersupplies. They don't all fail immediately some last longer than others yet 5 out of 5 have failed. They worked fine and regulated well enough when power is applied, I haven't had a chance to look at the waveforms but all of the circuits functioned properly so they must be pretty good and the output was within the expected range. Its just switching the power on and off kills. Vin,Vout, and GND are shorted  together after failure. Quick reply would be greatly appreciated. 

  • Look at the input voltage waveform at the IC.  That kind of hot plugging may cause the IC to see 2X the input voltage as a spike.  That may over voltage the IC.  If you need that kind of switch, you may want to put a TVS fron Vin to GND.

  • I thought this might be a problem. I reworked another board using a .5w 16v zener that is already on the BOM and it has worked after many make and break cycles without fail. This should only be hooked up to 12v systems so I would imagine it should be just fine or is there any other advantages to a TVS versus a Zener?
