TPS54620 Short Circuit

One of my customers is having problems with the TPS54620.

12V to 5V 5A

The TPS54620 is randomly dying in short circuit (I think it closes both top and bottom MOSFET at the same time, then overcurrent latch them up), after a while it delivers around 10V (I think it is when the bottom side MOSFET dies).

I attached the schematics for this design.

We found a workaround for now, but we need to understand what is happening.

The workaround is to remove FB14.

The customer wants to understand why this phenomenon is happening, and what to do against it.

    It seems FB14 is an EMI filter or ferrite bead. I'm not aware of any direct mechanism that would cause a TPS54620 device to fail, simply because it had an EMI filter on its input.

    Have they measured the VIN, SW and Boot waveforms during a short circuit condition, and compared to the abs max ratings in the datasheet? To assess what voltages are apparent at the pins of the device, these need to be measured using low-inductance probing methods, directly at the device pins.
