BQ20z45-R1 : Fuse blow without any apparent reason

Hello!

we are using TI BQ20z45-R1 together with a BQ294502 protector. we are doing COV, CUV, charge overcurrent and discharge overcurrent testing on the PCB itself. usnig a power supply to model the battery and IT was not enabled.

following which, we connected the battery and did a discharge and found that the fuse has blown. we have checked the EVSW and did not find any PF flags at all.

are there any possible reasons why the fuse blew? or prior to IT enable, could the fuse burn as well? the jumpers was connected and the PF were  set to FFFF.

sorry, we are worried that this situation might arise during production
