LM3410XMF Failure

We're using the LM3410XMF in our board to drive an LCD backlight. We had several failures and we're trying to understand what could cause the part to fail. One mode of failure is if the LCD is not connected and we power up the circuit. The other mode of failure if the input power to the LM3410 is being turned on and off rapidly with the LCD connected.

We're using a 15uH inductor (DO1608C-153) and two 1.24ohm resistors in parallel from FB pin to ground to increase the backlight brightness. The input voltage is 5V.

The LCD backlight specs are as follows Vled min=15V, Typ= 16V, Max =17.5V and the Current Min= 230mA, Typ 250mA, Max = 270mA.

What can cause this part to fail and are there any do's and don'ts when using this part?
