BQ2057 - charging circuit problem

Hi 

 we are using BQ 2057 for charging two cell lithium ion batteries each battery has (3.2V) for 7.4 V output

here i had attached the schematic of charger circuit , 

we are using 9v power supply for the circuit , after the circuit has powered up then i connected the battery then i found that the PMV 48Xp mosfet was heating up .

if prolonged for <30 sec it caueses the mosfet failure ie damage

why it was heating up.... and the battery also not charging

i measured it was giving 300 ma output after battery connected
