tps5410 issue when plug in, plug out many times with DC jack

Hi All 

My design like sample in user guide of TI, from 12v to 5v

I have issue when plug in and plug out power supply via DC jack many times.

Sometimes, have 5v ouput, sometimes it's about 0.6v.

Please help me to make clear this issue
