TPS62177 grenades on power up the second time, but works on the first.

ok so I am at a loss on this.

I have a small power adapter board to take 19V down to 3.3V.  when I apply power to it the first time, it powers up fine and produces 3.3v, however when i disconnect power and reconnect it, the chip literally blows a hole in itself by pins 2 and 3. (Voltage in, and Enable)


just to make sure i soldered a second one and verified no shorts between pins, and exactly the same behavior, first power on its fine, second one, POP + magic smoke.  the only deviation from the reference design is the output capacitor is 10uF rather than 22uF

I am leaving R1 open, and shorting R2 so it connects FB to ground for the 62177 chip

if it didn't work at all I wouldn't be as concerned but the fact that it works and then doesn't makes me worry it will grenade in the field also. 

I'm not even sure where to start on this.  The only thing i can think of is i didnt have any load connected to the output.
