Hiii.. 

            I need an urgent help regarding a DC DC power supply with input 18-60 Volt DC, Output 12V, 3 Amps using LM5088. I am not getting desired output and sometimes IC burns out around 40-60 Volt input..

I am attaching my Schematic and Layout here..
