LM5045 with eGAN drivers

Dear,

I would like to use the LM5045 with eGAN mosfets, therefore I need special drivers to drive them - LM5113. 

I would like to ask you how to connect correctly the drivers with the controller? How to terminate the unused pins: BST1,BST2, HS1,HS2? Just tie them to the ground?
