Blown out LM25011 when powered

I searched forum and internet about my problem but I didn't found any answer.


I used LM25011MY and input voltage is 36 V, output is 4,5 V / 1 A. You can see schematic below. My problem is sometimes LM25011 blow out when I powered. I designed a test setup for this problem. I powered circuit 10 second and I cut power 2.5 second than powered again. It is a loop. 10 circuit is paralleled and powered with CEN150-36 power supply. Tests samples output load was changing in test. I tracked output current of CEN150-36. The output current of CEN150-36 will increase maximum output current (~4,1 A) when some of test samples blown out. After same samples blown out, other will work properly.
I'm thinking about passive input current protection something like resettable fuse. Could you suggest solution or suggestion for this problem ?
