I am still working on my review of this design. I didn't want to just point out problems without providing you with proven suggestions on how to fix them. So I need a little more time to work it through.

Hello Chuck,

I'm testing on prototype board for +5V output that it is similar as reference design as link: www.ti.com/.../lm5026eval

There are some things wrong that can't output 5V. I will update the schematic later. This is very challenge to me, I'm investigating the mistake on prototype and in schematic.
