There is revised silicon available with enhanced reliability. Your schematic looks ok except the inductor value is lower than recommended and possibly too low current rating also. Usually the reliability issues have involved too high values for feed back resistors. Yours are low enough. It could also be related to poor layout of the PCB. What kind of failure did you observe? What were the conditions?
