TPS61070 failure -- SW is short to GND (dc resistance is about 30 Ohm)

Hi,

My design is using TPS61070 to provide +3.4V for system. There are two source for it: one is from battery, another is from USB+Regulator.

I use a diode and transistor to swithc between the two. I found TPS61070 will malfunction after several switch. Could you please tell me why?

Please refer to pic for schematics. Output current varies between 5mA and 70mA.
