TPS40211 - PMP3543 - Board not working as desired

Hi,

I have made a few boards of PMP3543 using the gerbers that are there on the T I website.  However, I am NOT able to get the boards working. When I gradually increase the input DC voltage (from a DC Power supply source), at around 10V, I get 52V output, which I feel is OK. However, when I further increase the input voltage gradually from 10V to 11V, 12V, 13V and so on, the output voltage begins to drop. Now, if I decrease the input DC voltage to 0 and then again gradually  increase it to 10V, I do NOT get any output. It is as though some component on the board has failed.  I observed this phenomenon on 3 boards! What could be the issue here ? 

When I got 52V output, the first time I powered ON the board, I observed the waveform at pin 1 of the device (TPS40211). Here I could observe a saw tooth waveform . Kindly advise as to how to get this board working again.PMP3543 Schematic_slvr917.pdf
