TPS7A1601 failed to short circuit input to output

We use the TPS7A1601 with an output voltage of 2.1V and an input voltage of 24V. We have had a few failures where the TPS7A1601 shows a short circuit between the input and output, and sometimes also between output and ground. Any ideas what could cause this? Is it more likely to be high voltage spike hitting the output or the input?

    Could you provide more information? Perhaps a schematic, or the load and temperature conditions of the device.

    Please do keep in kind that even though this a device with high thermal performance there might be certain limitations, please refer to this post for details on finding the device with the right thermals for your application.

    In your case, assuming maximum load of 100 mA, your device is dissipating around 2.2 W, which assuming an ambient temperature of 25°C leaves you looking at a RθJA of less than 46°C/W, met by the DRB (VSON) package, but not the DGN (HVSSOP) package, this will vary depeding on your ambient temperature.
