LM5576 - A hole is always formed in LM5576

LM5576- I made a LM5576 pcb which I got from the webbench Design center. The design was made to take an input voltage of 60V and output voltage of 12V with a maximum current of 2A.  For testing purpose, I was using  5 adapters of 12V  each in series to attain 60V. It is working fine in this scenario. When I try to connect it in the actual application environment, A hole is formed in the IC. What might have gone wrong here?

Is it the Voltage issue or the Heavy inrush current on cold start?


The hole is formed exactly below the number 7 and 6 LM5576 on the IC.


The 60V is supplied from a diesel generator.  Please do let me know if I am missing any protection circuits here.
