BQ24617 Typical Application Circuit and EVM board not charging.

I have a EVM board that won't bring the battery charge voltage up to charge 4 - 3.7V cells.  I pulled out R25 and changed from 909K to 698K. This seemed to work for a while and the battery will charge.  I then laid out the typical application circuit for the BQ24617RGET.  I did not use the FETs used in the typical but used the same FETs as the EVM board.  All other values are the same as typical application.  

Sometimes PG (Power Good) flashes rapidly and of course when I get the AC adapter voltage up the flash becomes solid on my board layout (this happens on the EVM sometimes and the voltage is sitting at 18V when it flashes on the EVM).  But the voltage to the battery will not come up to the 16.8V needed to charge the battery and I see maybe 7mA to the battery on my layout.  If  the AC Adapter (bench top supply) is turned off I will see 5.98V across the battery, then I turn on AC Adapter and the voltage drops to 2.9V across the battery.  If I set the AC adapter to 5V and turn on, the voltage across the battery will show 5.98V and as I increase the AC adapter voltage the voltage across the battery will increase and track with the AC adapter voltage until about 9V.  Then the voltage across the battery drops back down to 2.9V.

Any help would be much appreciated.
