UCD9244 POWER PROBLEM

Hi,

       We are using UCD9244 PWM contrller with two UCD7242 power driver  for generating the voltages 1V,1.2,3.3V & 1.8V . We have programmed the PWM controller  and the voltages are generated and worked for 2 months.

   All of sudden ,the second power driver which generates 3.3V & 1.8V is failed . I have connected the controller through TI fusion GUI and monitored the outputs  as 0.7V and 0.1V .  The fault bit is occured and i have cleared the bit. But the power driver never generates the voltage again.

   We have replaced the new power driver  IC and is working fine.

I would like to know what is  the failure cause   to avoid in future?
