I built 5 prototypes using a LMZ14201, 2 of the 5 have had a power supply failure (the output is shorted to ground)
Attached see schematic and layout (mid layers not shown, solid plane one for 3v3 and the other for ground)
When operating, the device runs barely warm. This power supply powers a back-lit TFT display, so I would expect the initial peak current draw to be significant, but then be less than 1A.
In both cases they worked initially and then failed upon application of power on a subsequent power-up
All capacitors are ceramic.
Do you see anything wrong with the schematic or layout?
Any suggestions on what to look for?
