TPS54625 is not Producing Output with Load/Without Load

The Circuit for generating 5.4V from 12V  using TPS54625 is Generated from WEBENCH and the  same circuit is used by me and the circuit is given below

After Giving 12V Input I am not seeing 5.4V(During No Load/ With Load) Output from the circuit shown above.

I have Captured the SS Pin signal through scope and shown Below

I have Captured the SW1 and SW2 Pin signal through scope and shown Below

Zoomed In Signal of SW1 and SW2 at Initial Time is Shown Below

Zoomed In Signal of SW1 and SW2 at middle time is Shown Below and observed irregular switching

Zoomed In Signal of SW1 and SW2 at irregular switching Instant

Zoomed In Signal of SW1 and SW2 at switching end is Shown Below and after that the signal slowly drops to 0V as shown in SW1 and SW2 Pin signal Full Capture.

Request you to suggest what may be the problem and Do you have any solutions for the problem?
