Review schematic and layout before send to UL test labs

Dear Sir,

I have power supply design that have been using UCC28740 and  the design is base on one of TI reference design.

Could you help me review schematic, layout of it and give me some comments to pass UL certification ?

    This is not a service generally provided by TI.  However, I will try to make some recommendations. 

    To meet UL the design needs to meet creep edge and clearance specifications.

    1. Primary to secondary 8 mm, 4 mm air gap.

    2. Isolation transformers must be design for reinforced isolation 4.5 kVA

    3. You can use component (FET, Diode) ratings and lead spacing's as guide to how far apart the traces should be.

    4. The PCB needs to be made of material that meets UL94V-0
